8109-B - Federal Tax Deposit Coupon
Form 8109-B is a blank Federal Tax Deposit Coupon coupon that is used to pay your withholding and payroll taxes when you do not have the preprinted 8109 coupon. Ordinarily, the IRS sends you the preprinted 8109 to make employer withholding tax deposits. However, it's been known to happen that an employer needs to deposit withholding taxes and is without their preprinted 8109.
Much like the 8109, you write in the amount of your tax deposit on the top of Form 8109-B. Then you have to write in your company name, address and employer identification number. Also darken the correct boxes for type of tax and tax period. Take the coupon to your federally chartered bank along with your check, payable to your bank.
If your quarterly withholding and payroll taxes are less than $2,500, you may mail a check when you file your Form 941. In this case, you do not need to use form 8109 or 8109-B.
