Form 8109 - Federal Tax Deposit Coupon

Form 8109 is a coupon that is used to pay your withholding and payroll taxes. The IRS will mail you a coupon book filled with these 8109 coupons after you have applied for your EIN-Employer Identification Number. You write in the amount of your tax deposit, the type of tax (usually 941 or 940) and the quarter that the taxes were withheld on form 8109.

Then write a check to your bank (yes, not to the U.S. Treasury) and take the coupon and check to a federally chartered bank.  If your withholding and payroll taxes are less than $2,500 for any quarter, you may mail a check with your quarterly Form 941. In this case, you do not need to use form 8109.
